This is something not commonly seen within the oesophagus of a 14 year old girl. She told me that it just 'slipped out of her hand'!
WHAT IS THE DIAGNOSIS?

That's what my patient told me and she wasn't lying but didn't tell me the whole story either ...
■ Help-seeking behaviour
Didn't come across as this. Once I removed it she was keen to go home with her mum
■ Borderline personality disorder
A possibility but the patient claimed that it was a simple mistake which anyone could have done ...
■ Something else?

explanation
By placing a snare around one end of this (it's the arm of a spectacle), it was possible to retrieve it. I was a little anxious because I wasn't using an overtube and if the patient had suddenly decided to flex her extended neck whilst the object was retrieved, she could have done some serious damage to her cervical oesophagus. Fortunately, she didn't move a muscle! Both her mother and I were amazed as to how it was even possible to swallow a 14cm object.
When I got home and told the story to my wife, who is a paediatrician, she asked me a simple question: " - did you have a look at her teeth?". Of course, the girl has bulimia and this was her way of making herself vomit. Not the first time my wife has made me look stupid. |
